Acer Nitro 5 AN515-42 (DRASTIC Performance Decrease - Stuttering, low fps)

Okay, Guys, 
This is my first post here and when I couldn't find much support online, I decided to come to the last resort. 
So I purchased this machine 2 years ago and it was a total beast at gaming. At least it worked well for me. 
I could play GTA 5 on high settings with an average of 60 fps.

I don't know what went wrong. I have been experiencing problems with gaming for almost a year now. I just can't play GTA without a stutter every second. It works only well if I scale it down to 720p with low settings. Weird right? As soon as I bump up my settings to just 1080 and low, things start to go crazy.
I'll be attaching some hardware monitor graphs and videos of the gameplay. 

I really need a solution to this as it makes my laptop completely useless now. I can not work properly on anything graphic intensive or play any game with moderate quality settings without a problem anymore.

Things I've tried - 
1. DDU and reinstalling the stock drivers from Acer.
2. Complete system reset - Factory Reset.
3. Trying different drivers from AMD's website.
4. AMD FreeSync on and off.

Can anyone please suggest me anything? I am really at my wit's end with this.
