Yep. My laptop is plugged in, NitroSense is at High-Performance, and High Performance is turned on in the windows power options.

That’s not a problem my friend it sometimes takes a little to get where we are but at least we have your GPU set up correctly and we know you are on High Performance with a full plugged in battery, which narrows down the performance of that particular game to in game rather than laptop settings. Go to your games website and their support section to find optimal game settings. Could you also run a couple of other games and report back on those

Right side of high performance there is change plan settings click on it

You'll be able to see Change advance power settings, proceed with that
4.)Scroll down and search for Switchable Dynamic Graphics and something there is one more setting
5.)Set both the options to Maximum Performance.
    Apply and Exit.

Just went to the graphics settings, clicked the 'classic' app option, browsed and added my game launchers to there. Then set them to high performance from there and now it seems to work a lot better. Been playing Dark Souls for roughly half an hour and the framerate has been pretty smooth and it hasn't kicked me out of online mode for low FPS. Also when I open Task Manager, click Performance and then GPU 0 (RX 560X GPU) it shows that it's being used.

Excellent, that is another way of achieving the same thing. Well done
