HDMI-ports showing different graphics cards. One does not work, help please! Acer predator G3-605

I have two hdmi outlets, one is glitchy and rarely works. When the glitchy one is working it is showing my graphics card as nvidia Geforce gtx 760 which runs games well, but when that outlet dosen't work it shows Intel HD graphics 4600, which runs games badly. I am asking if this is a computer problem, if the outlet is damaged, or if it is possible to get the nvidia card connected to the working outlet?

    ADD: I've had this problem for a couple months, i yesterday used discs to restore my computer to factory settings, and when i put the discs i could use the glitchy outlet(for the first time some weeks) and everything was working smoothly, until this morning when it stopped working again. 

    The monitor is just showing no signal.

    It sounds either like the drivers for the Geforce are getting updated to a bad version or the Geforce card is having problems. Check to see what version of the drivers is installed and roll back if you can. If they still match the version on the Acer support site it points to the card itself.

    @billsey How do i see what version of the drivers are installed, when i look under my graphics card i can only see the current drivers of the Intel HD graphics-card. I tried downloading the nvidia drivers but it said it couldent run.

    When the glitchy port is working, it'll be shown in Device Manager and you can check the driver version. When it's not working you might be able to see it by choosing to view hidden devices. One thing you might do when you're looking at the driver version is to uninstall them, telling it to remove all files. Then reboot and let Windows reinstall them. I'm leaning toward it being a hardware problem with the card or the slot, but if it's software those should help out.
