Hi, my spin 5 is about one year old but the wear is 22% according HWinfo64 (designed versus fully charged: 55964mWh versus 43228mWh. I'm leaving the Spin 5 charged allmost 24x7. What can be the reason for this extrem wear?

Corrupted data either in the battery or the OS. Both track battery usage and both work in concert with each other to give estimates for battery life remaining as you use the laptop. When they are using bad data, they give bad estimates. You likely want to do two steps to try and clear it up.
The first step is a battery reset, which has the battery start over with what should be good data. You don't tell us what model you have (it should be something like SP513-xxx-xxxx), but most of the newer ones have a battery reset pinhole on the bottom of the laptop. Shut down and disconnect everything. Turn the laptop over and use something like a bent paperclip to press and hold the reset button for 15-30 seconds. Release the button and wait for 15-30 minutes to make sure all residual electricity has bled off. Reconnect the charger and wait for the full battery indicator so show a full charge (typically it turns blue).
Next step is to do a battery calibration, or a few of them, in Windows. Turn on and log into Windows. Disconnect the charger and use your laptop normally until it is forced to shut down due to low battery. Depending on your power settings that might be at 10% left, or 5%, or 3%, or... Once it is off because of the low battery plug the charger back in and again wait until you get the full battery indication. That process trains Windows to better understand the battery discharge/charge cycle which helps it display accurate info for battery life. If you do the calibration several times in a row it will be much better.
Once these have been done rerun the battery status utilities to see where your wear level is actually at...
