Weird problem with graphic cards

Hi. I have a big problem and I don't know where to look for help anymore, so I refer the matter here. I have an Acer Nitro 5 laptop with NVIDIA GTX 1660TI graphics and an Intel I7 processor. I will illustrate the problem on the example of the fortnite game. So yes, when I set all the settings to low, the FPS in games is at 200-220 and after a few minutes of play it drops to 60-90. Playing on low settings can also stutter despite a large number of FPS. The case looks interesting, if I set the ultra settings, then the FPS jumps from 140-90 and the game is stable. Would anyone help me explain what's going on here? Why is the game on the lowest settings can cut and there are such huge drops in FPS and the ultra settings are like a fairy tale. I've already tried everything, ie set everything to maximum performance etc. Do you have any ideas?
