G245HQ + 7950 in Eyefinity boots, displays for 2s, then goes to black after bios screen

I have 3 G245HQ’s in eyefinity mode with a 7950. I am having an issue with one of the monitor intermitanly going to going black (blank screen, still on, can see images with a flashlight). This is how is always starts. Monitor works for a while, then goes black. I turn the monitor off and on a couple times. The monitor boots, displays the screen for 2s, and goes to black. I turn the monitor off for a couple hours, come back and most of the time it works then repeats this cycle later on.

 

I  sent the monitor in for repair twice. Both times they have found nothing wrong. I have switched DVI cables, display port adapters, etc, with no such luck. Any thoughts?

 

Windows 7 Ultimate,  Catalyst Version 13.9, Sapphire 7950

The Sapphire 7950 card has the following ports:

1x HDMI
1x DisplayPort
1x Single-Link DVI-D
1x Dual-Link DVI-I
On which ports do you have the 3 G245HQ monitors connected?

Is the "problem" monitor always connected on the same port?

For three monitors, I would suspect that you would need to connect them via HDMI, DisplayPort, and one DVI.
