Can't submit warranty case for XG270HU

I bought an Acer Monitor (model XG270HU) on Nov. 22, 2016. It has had a vertical white line on the right third of the display, which sporadically appears. The line is now present on the display all the time.

 

According to Acer's web-site, my monitor is covered by a "3 Year Carry In" warranty through 7/19/2019.

 

Today, I tried to submit a warranty case using this URL. But, when I get to the 3rd screen, titled 'Case Booking' the web-site won't let me go any further. I the 3rd screen I enter the SNID, Serial Number and attach my proof of purchase document. But, none of the other fields allow me to enter information into them, nor select from their menus. I tried clicking on the "Next" button, but it doesn't work either.

 

Does anyone know how to log a warranty case with Acer?
